Approved Exam Administrators can use their log-in credentials to sign
in here and gain access to forms and other materials
related to this program, including an on-line
Request For Tests page.
Using the log-in site is the best way to access
forms and manuals 24 hours a day! The on-line
Request For Tests form lets you complete and
electronically submit a request form at any time.
When you submit the request on-line you are able to
log back in to see the progress of the processing
and can track the UPS shipments of the exam
materials. If you are an Approved Exam Administrator
and do not have your log in credentials, please
